All-on-4 Dental Implants: Why Four Is Actually Enough

The Question Everyone Has Before They Ask It

Four implants. A full arch of teeth. It sounds like the math doesn't add up.

Most people assume more missing teeth means more implants, maybe a dozen, maybe more. So when they first hear about All-on-4 dental implants, the natural reaction is skepticism. Is this a shortcut? A compromise? A dental practice cutting corners somewhere?

It is none of those things. And once you understand the reasoning behind it, four starts to make a lot of sense.

Why Four Works: The Angle Is Everything

The key to All-on-4 is not just the number of implants. It is the placement.

In a traditional approach to replacing multiple teeth with implants, posts are positioned straight down into the jaw at even intervals. That works well when bone density is strong throughout. But many patients who have experienced significant tooth loss have also experienced some degree of jawbone shrinkage, because bone needs the stimulation of tooth roots to stay dense and full. When roots disappear, bone tends to follow.

All-on-4 is designed specifically with this reality in mind:

  • Two implants are placed toward the front of the arch, positioned vertically

  • Two implants are placed further back, angled up to 45 degrees

  • That angled placement allows the rear implants to find denser, more stable bone

  • The four together create a distributed support structure that holds the full arch securely

The engineering is deliberate. The angle is not a workaround. It is the point.

Who All-on-4 Is Actually For

This is where a lot of online information gets vague. All-on-4 is not a one-size-fits-all solution, but it does serve a wider range of patients than many people expect.

You might be a good candidate if you:

  • Are missing most or all of the teeth in one or both arches

  • Have been told you do not have enough bone for traditional implants

  • Are currently wearing a conventional removable denture and want something fixed

  • Have failing teeth that are being extracted and replaced in the same treatment plan

  • Want a permanent, non-removable result without the complexity of placing an implant for every single tooth

The right way to know for certain is a thorough evaluation, including imaging, to look at your bone structure, your bite, and your overall oral health. At East Madison Dental, that evaluation is where everything begins, whether you come to see us in Tenafly, Englewood, or Dumont.

What "Teeth in a Day" Actually Means (and What It Doesn't)

You will often see All-on-4 marketed alongside the phrase "teeth in a day." This is worth unpacking honestly.

In many cases, a temporary arch can be attached to the implants on the same day they are placed. You do not leave the office without teeth. That part is real.

What is also real is that the process is not finished that day:

  • The implants need several months to fully fuse with the jawbone, a process called osseointegration

  • During that healing period, you typically wear the temporary arch

  • Once integration is confirmed, your final permanent arch is placed

  • The final restoration is what you will wear long-term

So yes, the same day matters. But the full result is a process, and any honest provider will tell you that upfront. Dr. Narpat Jain, DMD, co-owner of East Madison Dental, practices implant dentistry across all three of the practice's locations and takes that same approach: walk patients through every stage clearly, no surprises, no overselling.

What Life Looks Like After

This is the part patients often want to know most, and it is also where All-on-4 tends to distinguish itself clearly from conventional removable dentures.

With a fixed All-on-4 restoration:

  • The arch does not come out at night

  • There is no adhesive, no slipping, no adjustment period every time you eat

  • You can eat most foods you would eat with natural teeth, with some initial dietary guidance during healing

  • You brush and care for it similarly to natural teeth, with some additional attention to cleaning under the arch

  • The implants continue to stimulate the jawbone, which helps preserve your facial structure over time

Patients frequently describe the change not just in terms of function but in confidence. Being able to speak, laugh, and eat without thinking about your teeth is something that is easy to take for granted until it is gone.

The Honest Conversation Worth Having

All-on-4 is not the right answer for every patient, and it is also not the complicated, out-of-reach procedure it is sometimes portrayed as. For the right candidate, it is one of the most meaningful things modern dentistry can offer: a fixed, permanent-feeling solution to significant tooth loss, built on fewer implants than most people expect and more science than most people realize.

If you have been putting off the conversation because it felt too big, too expensive to even explore, or too good to be true, starting with a real evaluation is how you find out what is actually possible for you.
